Privacy Policy
This policy explains what information Home Care Roadmap (“we,” “us”) collects, how we use and share it, and the choices you have. [Legal entity name to be inserted.]
Information we collect
- Care requests: ZIP code, type of care, timing, who needs care, expected payment source, your name, phone number, email, contact preference, and any notes you choose to add. We ask you not to include diagnoses, medications, or medical records.
- Consent records: when you check the consent box we store the date and time, your IP address, the page address, the version of the consent text you agreed to, and the agencies your request was shared with.
- Agency applications: business and contact details, license and certification numbers, service areas, and services offered.
- Newsletter: your email address and optional name, role, and state.
- Technical data: basic server and security logs (such as IP address and browser type) kept by our hosting provider.
How we use it
- To match and share your care request with up to 3 home care agencies serving your area, only if you consent.
- To confirm your request and tell you which agencies received it.
- To send the newsletter, if you subscribed and confirmed.
- To verify agencies, prevent fraud and spam, and operate and improve the site.
Who we share it with
- Home care agencies: up to 3 per request, only with your consent. Once an agency receives your request, its own privacy practices also apply.
- Service providers who help us run the site (hosting, form processing, email delivery), under contracts that limit their use of your data.
- When required by law or to protect rights and safety.
We do not sell your information to data brokers. We do not place advertising pixels or third-party trackers on our forms, and we do not share the contents of care requests with advertising platforms.
Health information
We are not a health care provider or health plan. We treat care requests as sensitive information: we collect as little as possible, limit access, and protect it with encryption in transit and at rest. Some state laws (for example, Washington’s My Health My Data Act) give you additional rights over consumer health data, and we honor those rights.
How long we keep it
We keep care requests for [retention period] and consent records for as long as needed to demonstrate consent under applicable law, then delete or de-identify them.
Your choices and rights
- Withdraw consent or stop texts at any time (reply STOP) or by emailing us.
- Unsubscribe from the newsletter using the link in any email.
- Request access to, correction of, or deletion of your information by emailing [email protected]. We will respond as required by applicable law. We can’t delete information already held by agencies you were matched with, but we’ll tell you who they are so you can contact them.
Security
We use reasonable administrative, technical, and physical safeguards. No method of transmission or storage is completely secure.
Children
This site is intended for adults. We do not knowingly collect information from children under 13.
Changes
We will post updates here and change the effective date. Material changes to how we share care requests will be highlighted.
